Change in Enrollment Request Form

We love having you in our classes and will be sad to see you go, but we also understand that sometimes change is necessary. Please share below how we can best assist you in this change. If you need a schedule change, we are happy to help you find a place in another class. We truly appreciate any feedback you can give about your enrollment change request.

Requesting a Change in Enrollment:

-Fill out the form below by the 25th of the month. Only changes made by the 25th will avoid charges the next month. No refunds will be given once payment runs for the month on the 1st.

-Look for an email from our office indicating we received your change in enrollment form below. Requests made by email for change in enrollment will not be accepted and you will be directed to fill out this form.
